SMS Terms & Conditions

These SMS Terms & Conditions apply to text messages sent by OPTMZ, LLC d/b/a OPTMZ Health ("OPTMZ Health," "we," "us," or "our") to the mobile number you provide. By opting in, you agree to these terms, our Terms of Use, and our Privacy Policy.

Program Description

By providing your mobile phone number and opting in through our website forms, consultation requests, checkout, or service inquiries, you consent to receive SMS messages from OPTMZ Health related to reminders about your upcoming consultations, follow-ups regarding continuing care, treatment updates, prescription or order and shipping notifications, account and security messages, and messages related to ongoing care and services. Messages may be sent using automated technology. Consent to receive SMS messages is not a condition of purchase.

Opt-In Message

When you opt in, you may receive a confirmation message similar to: "OPTMZ Health: You have successfully opted in to receive SMS messages regarding consultations, care updates, and service notifications. Message frequency may vary. Msg & data rates may apply. Reply STOP to opt out or HELP for help."

Message Frequency

Message frequency may vary depending on your level of engagement with OPTMZ Health. You may receive consultation reminders, follow-ups, order updates, and important care-related notifications.

Message and Data Rates

Standard message and data rates may apply depending on your mobile carrier and service plan. You are responsible for any charges from your carrier.

Opting Out

You may opt out of SMS messages at any time by replying STOP to any message. After opting out, you will receive one final confirmation message, and we will no longer send SMS messages to your number unless you opt in again. Opting out of SMS does not affect other communications related to your care, such as email.

Eligibility and Your Number

You must be at least 18 years old to opt in. You confirm that you are the account holder for the mobile number provided, or that you have the account holder's permission. Please notify us if you change or deactivate your number.

Health Information and Security

Text messaging is not a fully secure method of communication. We limit the health information included in SMS messages. Do not use SMS to report a medical emergency — if you are experiencing a medical emergency, call 911.

Privacy

Your mobile number and SMS opt-in consent will not be sold or shared with third parties or affiliates for their marketing purposes. Information is handled as described in our Privacy Policy and HIPAA Notice.

Carrier Liability Disclaimer

Mobile carriers are not liable for delayed or undelivered messages.

Changes to These Terms

We may update these SMS Terms from time to time. Continued participation in the SMS program after changes are posted means you accept the updated terms.
